Below find guidelines on how to use our services.
This process is usually initiated from the ordering provider, but if the provider chooses, he/she may issue a valid order to the patient and then they may call to schedule themselves personally through Central Scheduling at 910.671.5594 from 7:00am-6:00pm Monday thru Friday.
You should provide the following information:
- Physician's name and telephone number
- Test or procedure
- Diagnosis (diagnosis can’t include – “rule out”, “to evaluate”, or ”suspected”)
- Suggested time and date of service
Each visit to SRMC will require a new registration. Patients are asked to provide specific information to ensure the accuracy and integrity of each registration. Patients who have pre-registered should report directly to the department for their test or procedure. Pre-Admission Testing patients should report to Registration. If at any point you are unsure of a location or your registration status, stop by the Registration department located near the main lobby. Hostesses are available to assist you.
You should bring the following items with you when coming for pre-registration and on the day of your test or procedure:
- All current medical insurance cards
- A photo ID
- Any paperwork from the ordering physician
- Living Will and Power of Attorney information
If you are under 18, a parent or legal guardian must be present to sign consent forms before services are rendered in most cases.
If your physician has scheduled an appointment for pre-admission testing, you should report to the registration area. Pre-Admission testing patients are scheduled to arrive 15 minutes before their appointment.
Access our web-based pre-registration menu from the convenience of your computer. Procedures must be scheduled 2 business days prior to the registration to ensure that all information is processed accurately.

For a more personalized registration experience, visit our office on the main campus. Hours of operation are Monday through Friday from 7:00am to 6:00pm. The main campus is located at 300 W. 27th Street Lumberton, NC 28358. Registration is located off the main visitors lobby.
