Diabetes Community Center
Diabetes Community Center provides a nationally recognized outpatient diabetes self-care training program; physician referral required.
Southeastern Health’s Diabetes Community Center (DCC), which is recognized by the American Diabetes Association, serves individuals of all ages who have diabetes by providing in-depth education to help manage their disease and prevent complications.
The DCC team, which is made up of certified diabetes educators, certified diabetes dietitians and registered pharmacists, works with patients to improve their diabetes condition, whether Pre-Diabetes, Type 2, Type 1 or Gestational Diabetes. With guidance from the team, those with diabetes, including children, can learn to manage their disease through diet, exercise and medication. Education includes hands-on training for meters, insulin injections, continuous glucose monitoring system (CGMS) applications, insulin pumps as well as carb counting and meal planning.
DCC team members are located at SeHealth affiliates throughout the region as well as centrally located in Lumberton. Ask your provider if a referral to Southeastern Health’s Diabetes Community Center would help you gain control of your diabetes today.
